    甲亢期桥本甲状腺炎和毒性弥漫性甲状腺肿的鉴别诊断

    摘要：桥本甲状腺炎（Hashimoto’s thyroiditis，HT）和毒性弥漫性甲状腺肿（Graves disease，GD）是两种常见的自身免疫性甲状腺疾病（Autoimmune Thyroid Disease，AITD）。在这篇综述中，我们使用 “甲亢期 HT” 来指代早期 HT 伴有甲状腺功能亢进的临床表现。在临床实践中，由于甲亢期 HT 和 GD 表现出非常相似的临床症状，因此不容易区分。现在文献缺乏从各方面系统地比较和总结甲亢期 HT 和 GD 所致的甲状腺功能亢进的研究。重视甲亢期 HT 和 GD 的各种临床指标的对比，做到清晰明确的鉴别诊断十分必要。我们在 PubMed、中国知网、万方、维普等数据库检索了甲亢期 HT 及 GD 的相关文献。对相关文献中提取的信息进行总结和进一步分析。鉴别诊断甲状腺机能亢进的病因是甲亢期 HT 还是 GD，建议第一时间关注血清学检查，然后是影像学检查，以及甲状腺 I131 摄取率检查。在病理学上，细针吸细胞学（fine needle aspiration cytology，FNAC）是鉴别甲亢期 HT 和 GD 的金标准。细胞免疫学和基因遗传学方面的检测结果也可以用于两种疾病之间的准确诊断，未来可能会进一步开发和研究。本文从血液检查、影像学、甲状腺 I131 摄取、病理、细胞免疫学、遗传学等六个方面，对鉴别诊断甲亢期 HT 与 GD 作以综述。
